About the role

Key responsibilities & impact
  • Provide ad hoc support and direct care to patients and families
  • Establish and maintain positive relationships with team members, patients/families, and referral resources
  • Provide and manage direct care incorporating psychosocial, spiritual, cultural, physical, and biological components
  • Handle clinical inquiries related to Chapters Health System’s services
  • Promote and explain the benefits of Chapters Health System’s services
  • Gather necessary information and documentation for complete, current, and accurate patient records
  • Enter data into information tracking systems and the electronic medical record (EMR)
  • Provide reassurance to patients and families
  • Coordinate or complete in-person visits when needed or requested
  • Assist patients and families in achieving desired comfort levels
  • Use appropriate support and expert resources to resolve complex situations
  • Document patient/family contacts in the EMR and communicate with the Interdisciplinary Group (IDG)
  • Report to a company-designated location during emergencies to ensure continuity of services, potentially ahead of schedule and overnight
  • Perform other duties as assigned

Requirements

What you’ll need
  • Current license as RN in the state where the employee will be working
  • Minimum of one (1) year nursing experience; hospice or hospital experience preferred
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
  • Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred
  • Excellent critical decision making and problem-solving skills
  • Excellent clinical and assessment skills
  • Ability to use equipment with visual and auditory mechanisms
  • Mobile Driver – valid driver’s license and automobile insurance per Company policy
  • Excellent time management skills with the ability to prioritize demands to meet patient service standards and deadlines
  • Ability to effectively handle stressful situations professionally and calmly with exceptional customer service
