About the role
Key responsibilities & impact- Provide independent primary care for older adult patients with acute and chronic illnesses
- Take full accountability for patient care and outcomes and seek specialist consultation when needed
- Engage with hospitalists when patients are hospitalized
- Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document patient needs
- Lead care teams consisting of care promoters, care facilitators, and care coordinators
- Coordinate with transitional care teams, case managers, acute and transitional-care physicians, and other resources for patients unable to visit the office
- Participate in center management and cover for absent providers
- Recruit patients for the center and additional providers for the company
- Supervise and collaborate with Advanced Practice Practitioners and remain accountable for their actions
- Participate in risk and quality management programs, clinical meetings, and other required meetings
- Work toward patient admissions, appointment volume, patient experience, clinical gap closure, and medical cost goals

Requirements
What you’ll need- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
- Board Eligibility is required
- Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employment
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook
- Ability and willingness to travel locally as needed, nationally for initial training, and occasionally regionally and nationally for recruiting or training
- Fluency in English, verbal and written
- Ability to use independent judgment
- Flexible to work evenings, weekends and/or holidays as needed
