About the role
Key responsibilities & impact- Coordinate delivery of care within the Neuromuscular and Muscular Dystrophy Clinic and across healthcare settings
- Assess the physical, functional, and psychosocial well-being of patients and families
- Assist patients with transition to adult healthcare services
- Develop, implement, and evaluate collaborative plans of care
- Communicate with, educate, and engage patients, caregivers, and providers across care settings
- Provide health teaching and health-promotion education tailored to learning needs, abilities, readiness, culture, spirituality, language preferences, and barriers
- Use health literacy strategies, formats, tools, approaches, and technologies to deliver care
- Contribute to clinical practice guidelines, policies, procedures, and continuous quality improvement
- Advocate for patients and caregivers and help resolve complex ethical issues and inequities in care
- Provide telehealth care management and coordination, including telephone triage
- Support care coordination and transition-readiness initiatives
- Assist the clinic coordinator with specialized prior authorizations for multimillion-dollar gene therapies
- Contribute to program growth and development and ensure an exceptional patient and family experience
Requirements
What you’ll need- Associate's Degree in Nursing required
- 2+ years Licensed nursing experience required
- BLS-Basic Life Support Healthcare Provider certification required from the American Heart Association, American Red Cross, or National Safety Council
- Current Registered Nurse license: State of Wisconsin RN license or Tennessee Compact RN license
- Proficient pediatric assessment skills using a developmental approach across neonatal, infant, toddler, preschool, school-age, adolescent, and young adult patients
- Ability to work with patients with intellectual and physical disabilities in a developmentally appropriate manner
- Proficiency with computers and an electronic medical record
- Independent judgment and analytical ability to problem solve, make decisions, and formulate reports
- Effective collaborative interpersonal and communication skills for discussing sensitive issues with patients and caregivers
- Ability to represent the organization professionally and promote a positive image of the organization and its services
- Expertise in active listening and therapeutic communication
- Critical reasoning and astute clinical judgment
- Must be available to travel to all closer-to-home sites in addition to the main campus a couple times a month
