About the role
Key responsibilities & impact- Serve as the primary pharmacist supporting the Swank Center for Memory Care
- Collaborate with neurologists, advanced practice clinicians, nurses, and other healthcare professionals
- Optimize medication therapy and improve patient outcomes
- Enhance the patient and caregiver experience
- Provide comprehensive medication management
- Educate patients and caregivers
- Provide medication access support
- Monitor patients receiving therapy for memory and cognitive disorders
- Support additional specialty and ambulatory clinical pharmacy services based on patient care needs, program growth, and organizational priorities
- Build relationships with patients and interdisciplinary teams
Requirements
What you’ll need- PharmD from an ACPE-accredited school of pharmacy required
- ASHP-accredited PGY1 Pharmacy Residency required or a minimum of 3 years of relevant ambulatory clinical pharmacy experience
- ASHP-accredited PGY2 Pharmacy Residency preferred
- Board certification preferred
- Active, unrestricted licensure as a Pharmacist in the State of Delaware required prior to hire
- Active, unrestricted licensure as a Pharmacist (or eligible to reciprocate) in additional states where patient care is provided required or obtained within 2 years of hire, as applicable
- May require intermittent local or regional travel to support specialty clinics, meetings, or multi-site responsibilities
