About the role

Key responsibilities & impact

Provide care to medical-surgical patients in their homes through the Hospital Care at Home Program.
Deliver hospital-level care in patients’ homes rather than in a hospital.
Treat and monitor patient health.
Participate in a care team comprising physicians, nurse practitioners, physician assistants, paramedics, and registered nurses.
Provide or coordinate in-person home visits to patients hospitalized at home, including two visits per week.
Conduct virtual visits.
Monitor patients through the Telehealth Platform.
Support services including laboratory, infusion therapy, wound care, imaging, PT, OT, EKG, advanced practice provider in-home visits, virtual specialty provider consults, and medication delivery.

Requirements

What you’ll need

RN License in Delaware or a Compact State
BSN preferred or committed to acquiring BSN within 3 years of the hire date.
A minimum of three years of med/surg experience as an RN is required.
Critical Care experience is a plus.
BLS required.
Active Drivers License
Ability to stand, walk, sit, bend, lift, push, pull, carry, reach, twist, handle, stoop, and climb as per practice area-based Physical Demands Checklist.
Frequent exposure to biological elements with occasional exposure to chemical and environmental elements as per practice area-based Physical Demands Checklist.
