About the role
Key responsibilities & impact- Provide community-based care coordination for children and families experiencing serious emotional disturbance, trauma, or co-occurring behavioral health disorders
- Engage newly referred youth and families in care coordination
- Conduct in-person meetings with clients and caregivers within required timeframes
- Develop Child and Family-Centered Care Plans and identify client family teams
- Facilitate Child and Family Team meetings at least monthly or as needed
- Complete comprehensive assessments, including CANS assessments
- Develop safety and crisis plans
- Facilitate referrals and linkages and monitor care plan implementation
- Manage 1915 Waiver funds and obtain prior authorization and ODM approval
- Ensure documentation meets requirements and bill for care coordination activities
- Perform discharge and transition planning
- Coordinate care with providers, agencies, physical health providers, and school personnel
- Monitor CFCCP implementation, team commitments, and service effectiveness
- Document team attendance, approvals, and signed CFCCPs in the youth’s EHR
- Provide CFCCPs to Aetna for quality review and initiate service authorization processes
- Spend most working hours in the community meeting clients and conducting child family team meetings; work onsite at CCHMC Vernon Manor campus when not in the community
Requirements
What you’ll need- Bachelor’s degree plus 2 years’ experience, or Master’s degree with 1 year experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
- Must have reliable transportation
- Must reside in the Scioto/Lawrence County area
- Wraparound experience
- Meeting facilitation experience
- Community-based experience
