About the role
Key responsibilities & impact- Evaluate, diagnose, and manage adult patients with hypertension, chronic kidney disease, kidney stones, and electrolyte disorders
- Provide care across the West Region in outpatient clinics and outpatient hemodialysis settings
- Work collaboratively with physicians and multidisciplinary team members to optimize patient outcomes and deliver high-quality kidney care
- Conduct medical histories and complete physical examinations where indicated
- Initiate and interpret laboratory, radiology, cardiographic, and other diagnostic tests as credentialed and privileged
- Establish working diagnoses and treatment plans
- Perform routine and specialized procedures as credentialed and privileged
- Prescribe and monitor medications as permitted by licensure, state boards, and Cleveland Clinic policies
- Educate patients about diseases, treatments, medications, side effects, and hazards
- Participate in medical team rounds and coordinate with nursing, medical, and other healthcare team members
- Refer patients to specialists and specialized health resources as appropriate
- Serve as a liaison to ensure continuity of care across multidisciplinary and specialty teams
- Review records and treatment plans to monitor quality and patient outcomes
- Facilitate appropriate length of stay, discharge planning, and regulatory compliance for inpatient management
- Support professional development of the healthcare team
- Promote translation of evidence-based practices, guidelines, and research
- May conduct research studies and assist with data collection and analysis
- May precept students and onboard new caregivers
- Occasional early evening dialysis rounding may be required
- Perform other duties as assigned
Requirements
What you’ll need- Nationally certified Advanced Practice Provider (NP/PA/CNS)
- APRN: Graduate of an accredited APRN program
- APRN: MSN required upon hire or within 3 years of hire for applicable external hires and internal hires moving from a non-APRN licensed role
- PA: Graduate of an A.R.C.-approved Physician Assistant Program
- PA: Master’s degree in Physician Assistant Studies and/or higher degree obtained from a program recognized by the practicing state’s Licensing Board
- NP: Currently licensed as a Nurse Practitioner in applicable state(s)
- NP: Must be licensed as an RN in applicable state(s)
- NP: National certification as a Nurse Practitioner by a nationally accredited organization recognized by the State Board of Nursing
- CNS: Currently licensed as a Clinical Nurse Specialist in applicable state(s)
- CNS: Must be licensed as an RN in applicable state(s)
- CNS: National certification as a Clinical Nurse Specialist by a nationally accredited organization recognized by the State Board of Nursing
- PA: Certification as a Physician Assistant through the NCCPA or successor organization recognized by the practicing state’s Licensing Board
- PA: License to practice and prescribe issued by the practicing state’s Licensing Board
- PA: Current DEA License or DEA designee as required by NCQA
- Basic Life Support (BLS) through the American Heart Association (AHA) required
- ACLS, PALS, NRP, or ATLS may be required based upon work setting
- NP/CNS: Arrangements with at least one physician in accordance with applicable State Board of Nursing requirements
- PA: Physician Supervision Agreement and signed addendum required
- Credentialing and privileges through the Medical Executive Committee and Governing Body of applicable hospitals
- Oral and written communication in English
- Excellent analytical and decision-making skills
- Strong written and verbal communication skills
- Ability to work as a member of a multidisciplinary team
- Demonstrated competency in advanced assessment and diagnosis
- Demonstrated ability to use the electronic medical record
- Critical thinking skills, decisive judgment, and ability to work independently
- Ability to work in a stressful environment and take appropriate action
- Related clinical experience preferred
- Ability to walk and stand for long periods
- Manual and finger dexterity and eye/hand coordination for physical examinations
- Corrected vision and hearing within normal range
- Ability to occasionally lift and carry items weighing up to 50 pounds
- May require exposure to communicable diseases and/or body fluids
- Must follow standard precautions and use required personal protective equipment
