About the role

Key responsibilities & impact
  • Coordinate the care and services of select patients
  • Intervene with patients who have complex psychosocial and discharge planning needs
  • Conduct detailed psychosocial assessments and coordinate with community resources and supportive counseling
  • Provide crisis intervention to patients and families and facilitate transitions of care for high-risk patients
  • Manage discharge planning and connect patients with post-discharge care providers
  • Work with families exhibiting complex family dynamics
  • Communicate discharge planning status to the interdisciplinary team
  • Educate patients, families and physicians regarding post-acute options, freedom of choice and financial concerns
  • Conduct ongoing psychosocial assessments and interventions and make treatment recommendations
  • Address social, psychological, financial, behavioral and regulatory barriers to successful medical outcomes
  • Provide counseling and intervention related to treatment decisions and end-of-life issues
  • Advocate for patients and families in cases involving abuse, neglect, domestic violence and sexual assault
  • Empower patients and families to make autonomous healthcare decisions and access needed services
  • Maintain social work competencies and regulatory compliance
  • Complete mandatory and continuing education requirements
  • Serve as a preceptor, mentor and resource to less experienced caregivers
  • Represent the department on committees

Requirements

What you’ll need
  • Master’s degree in social work (MSW) or Master of Science in Social Administration (MSS)
  • Current state licensure as a Social Worker (LSW, LISW or LISW-S)
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) or the American Red Cross
  • Flexible, open-minded and adaptable to change
  • Certification as a Case Manager within one year of eligibility from an approved professional organization (preferred)
  • Three years of experience in a healthcare system including exposure to and experience in pre- and post-acute care (preferred)
  • Hospital social work experience (preferred)
  • Knowledge of community services (preferred)
  • Ability to meet physical requirements, including extensive standing and walking, normal or corrected hearing and vision, and sedentary work requirements
  • Compliance with personal protective equipment requirements, drug testing, and influenza prevention program
