About the role

Key responsibilities & impact
  • Facilitate and ensure effective transitional care coordination across the continuum of care
  • Advocate for patients and perform holistic assessments
  • Develop care plans and empower patients and families to self-manage
  • Coordinate patients’ next level of care
  • Ensure compliance with accrediting-body standards, revised state code and CMS Conditions of Participation
  • Promote interprofessional synchronization throughout the acute episode
  • Complete effective handoffs to the next level of care or community
  • Assess, identify and mitigate barriers to patients’ transitions of care
  • Develop comprehensive, individualized transitional care plans with patients and care partners
  • Participate in and lead patient rounding activities, huddles and committee meetings

Requirements

What you’ll need
  • Graduate from an accredited school of professional nursing (Diploma, ADN or BSN program)
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross
  • Three years of recent RN clinical experience
  • Ability to work six weekend shifts per six-week period, including Friday, Saturday, Sunday and Monday shifts from 8:00AM–4:30PM
  • Ability to comply with controlled-substance testing after an offer of employment
  • Ability to comply with the annual influenza prevention program
  • Normal or corrected hearing and vision
  • Ability to perform work in a stationary position for extended periods
  • Ability to communicate and exchange accurate information
  • Ability to work with physical records and operate a computer and other office equipment
  • Ability to travel throughout the hospital system
  • Preferred: Bachelor of Science in Nursing (BSN)
  • Preferred: Certification by an approved entity within one year of eligibility (CTM, ACM or CCM)
  • Preferred: Two years of recent experience in utilization review, quality or care management
  • Preferred: Knowledge of CMS regulations
