RN Care Coordinator – Orthopedics Trauma

About the role
Key responsibilities & impact- Work collaboratively with multidisciplinary care team staff across the continuum of care for high-risk patients
- Provide longitudinal care and disease-management coordination for chronic conditions or episodic surgical care
- Conduct patient outreach and care coordination for a panel of patients
- Develop goals, plan interventions, and maximize patient outcomes
- Identify patients with ongoing coordination needs and conduct targeted outreach
- Conduct comprehensive clinical assessments covering disease/age-specific, medical, behavioral, pharmacy, social, and end-of-life needs
- Educate and coach patients and families regarding care coordination
- Monitor patient compliance with care plans and reassess progress toward goals
- Update plans of care
- Serve as a liaison and advocate for patients and families
- Assist with transitions of care across settings through communication and planning
- Identify barriers, facilitate solutions, and connect patients to community resources
- Work Monday through Friday, primarily 7:30am–4:30pm
- Be stationed at Fairview and float to Richard E. Jacobs, Westlake, and Lorain facilities
Requirements
What you’ll need- Graduate from an accredited school of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
- Three to five years of nursing experience
- Registered nurses or advanced practice nurses working in designated oncology settings or administering chemotherapy must obtain cancer-specific certification or demonstrate ongoing qualifying education within the facilities’ accreditation cycle
- Preferred: Bachelor’s of Science in Nursing (BSN)
- Preferred: Specialty certification
- Ability to meet physical requirements, including full range of motion, manual and finger dexterity, eye-hand coordination, and corrected hearing and vision
- May require exposure to communicable diseases or bodily fluids
- Must follow Standard Precautions and use personal protective equipment as required
- Successful controlled-substance testing following an offer of employment
- Must comply with Cleveland Clinic’s influenza prevention program
