About the role
Key responsibilities & impact- Deliver professional and ethical care to patients
- Support the patient pathway from hospital to home through home visits and community clinics
- Plan, manage, and organise caseload and time effectively and efficiently
- Communicate with patients, carers, and referring healthcare professionals and provide feedback
- Undertake Appliance Use Reviews (AUR) where appropriate according to company and Department of Health guidelines
- Maintain clinical records in accordance with company, legal, and professional requirements
- Establish working practices that support collaboration between acute and community healthcare teams
- Build and maintain effective relationships with healthcare professionals across the region
- Deliver educational support to fellow healthcare professionals

Requirements
What you’ll need- Experienced Nurse with relevant clinical expertise
- Registered General Nurse
- Expert clinician in Stoma management
- Sound interpersonal and relationship building skills
- Teaching experience
- IT proficiency
