About the role
Key responsibilities & impact- Provide high-quality primary care in a high-touch, person-centered care model
- Assess, diagnose, and manage chronic and acute conditions
- Provide health education
- Develop and communicate treatment plans to residents, providers, and caregivers
- Order and interpret laboratory and diagnostic testing and adjust treatment plans
- Maintain compliant documentation in the EMR
- Collaborate with PCPs, interdisciplinary care teams, operations, and clinical leadership
- Support value-based care initiatives
- Provide excellent customer experience
- Follow professional standards, board certification requirements, and state and federal regulations
- Coordinate care with providers, specialists, and telemedicine providers
- Complete required monthly, quarterly, annual, and acute visits
- Participate in family conferences and advanced care planning
- Address pharmacy recommendations and pharmacist communications
- Be present at assigned facilities during work hours for face-to-face resident and facility coverage
- Service Hawthorne Healthcare Center three days per week and Evergreen Crossing & Lofts two days per week
- Work first shift, Monday through Friday, with no on-call, weekend, or holiday coverage
Requirements
What you’ll need- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification of AANP or ANCC
- Active, unrestricted state license
- Active DEA license
- US work authorization
- 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting
- Ability to work in a flexible environment and schedule
- Passion for providing healthcare to senior and low-income populations
- Strong oral and written communication skills
- Detail-oriented with ability to work independently and in a team setting
- Customer service approach with teams and residents
- Strong time management skills and ability to prioritize competing deadlines
