About the role

Key responsibilities & impact
  • Apply a person-centered approach to keep the client at the center of all program functions
  • Coordinate the day-to-day implementation of contractual requirements within the state’s Medicaid guidelines
  • Create care plans based on clients’ needs, train clients on services provided, audit services, and report findings to the contracted entity
  • Assist clients with plan/budget development, submissions and revisions
  • Provide service to clients, employees, and third-party organizations including MCOs
  • Support day-to-day operations and client advocacy
  • Collaborate with stakeholders regarding delivery of services
  • Conduct advanced reporting and oversight activities
  • Handle complex service programs and special projects
  • Develop and implement care plans
  • Enroll clients and employees in services, including development of service/support plans and budgets
  • Orient and educate clients on program roles and responsibilities
  • Train and assist clients in timecard submission
  • Assist clients with eligibility requirements
  • Travel to and from client homes or other destinations to meet with clients
  • Conduct necessary client assessments and contacts
  • Maintain documentation in the CRM database
  • Compile information for internal or external auditors
  • Market services to referral sources
  • Represent the company at stakeholder meetings, health fairs, and provider fairs
  • Supervise deliverables and ensure timely completion of projects
  • Participate in professional development and training activities
  • Provide customer service to internal and external clients
  • Perform other duties as assigned

Requirements

What you’ll need
  • Bachelor’s degree in the field of Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field
  • 6 years’ experience serving individuals with disabilities
  • Must have reliable transportation
  • If the employee will be driving, a valid driver’s license and auto insurance is required
  • Previous Support Broker experience preferred
  • Combination of education and experience
  • Ability to work flexible and/or extended hours, if needed, to meet the job requirements
  • Employees must provide their own reliable internet if working a remote/hybrid position
  • Legally authorized to work in the United States
  • Consumer Direct Care Network does not provide visa sponsorship or STEM OPT extensions
  • Bilingual preferred in various locations
