About the role
Key responsibilities & impact- Connect with patients monthly to address needs and provide resources
- Connect and maintain relationships with community partners for patient resources
- Work closely with a dedicated team to provide exceptional patient care
- Participate in community awareness and engagement events promoting Crossroads, its mission, principles, and addiction-treatment approach
- Work from the primary location in Wilkinsburg and secondary locations in North Side and Bloomfield
- Travel between secondary work locations
Requirements
What you’ll need- At minimum, High School Diploma or GED required
- Experience in a fast-paced environment, servicing many clients, preferred
- Experience in healthcare, addiction medicine, community health, or care coordination preferred
- Experience with building individualized care plans is a plus
- Knowledge of compliant documentation practices
- Lived Addiction experience is a plus
- Ability to work 36–40 hours per week
- Ability to work at primary and secondary locations in Wilkinsburg, North Side, and Bloomfield
- Ability to travel between work locations
