About the role

Key responsibilities & impact
  • Provide individual counseling to highly complex patients
  • Conduct clinical assessments to assess patient treatment needs
  • Design and update patient treatment plans, including care coordination, referrals, and discharge planning
  • Manage assigned caseload and compliance with clinical treatment requirements
  • Collaborate with nurses and providers to provide holistic treatment services
  • Facilitate urine drug screening for patients

Requirements

What you’ll need
  • General experience working with underserved populations in a clinical setting
  • Lived addiction experience is a plus
  • Qualified candidates will hold one or more of the following certifications/licenses: LMSW, LCSW, LPC, or LADAC
  • Expected hours of 40 hours per week
  • Ability to work 8:00 AM–5:00 PM, Monday through Friday
