About the role

Key responsibilities & impact
  • Direct, administer, and coordinate clinical resident care services
  • Lead, manage, and hold overall accountability for the wellness team and its performance
  • Manage Wellness revenue, evaluations, care conferences, and service plans
  • Ensure the right care, at the right price, with the right staff
  • Serve as Alzheimer and Dementia Program Coordinator
  • Lead according to 1440 care standards
  • Translate organizational, community, and wellness goals for team members, staff, and residents
  • Build partnerships with community leaders and external skilled-care partners
  • Market wellness throughout the community and build visibility, credibility, and trust
  • Perform resident admissions and ongoing evaluations every six months, after hospital or skilled-environment returns, and upon changes in condition
  • Establish measurable resident goals and outcomes addressing physical, mental, and psychosocial needs
  • Manage state regulations and audits to maintain community compliance
  • Obtain written orders for changes to care plans
  • Verify MARs/TARs and communicate with the pharmacy regarding medication and treatment administration accuracy
  • Review incident reports and determine whether assessments or care conferences are needed
  • Address identified personnel issues
  • Communicate emergency policies and provide updated information to staff
  • Work toward continual improvement of the organization
  • Perform other duties as assigned

Requirements

What you’ll need
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) registered within the State required
  • CDP required prior to or upon hire within 90 days
  • Assisted Living/Memory Care experience preferred
  • Leadership experience preferred
  • Proven ability to manage revenue and financials successfully
  • Proven high performer
  • Ability to work successfully through complex issues and problem solve
  • Available to work a flexible schedule including on call, weekends, and holidays when necessary
  • Ability to communicate effectively, sit, stand, walk, use hands to handle keyboard, telephone, paper, files, and other equipment and objects
  • Ability to review detailed documents and read computer screens
  • Ability to occasionally lift and/or move up to 25 pounds
  • Occasional travel to different locations may be required
