About the role

Key responsibilities & impact
  • Support improvements in primary care in family practice settings
  • Develop quality improvement capacity across practice teams through tailored practice facilitation
  • Use quality improvement methodology, coaching, facilitation, and change management to help doctors adopt and sustain Patient Medical Home attributes
  • Work as part of a support team including Health Technology Coaches, Panel Management Coaches, peer mentors, and program staff
  • Collaborate with Divisions of Family Practice, health authorities, the Ministry of Health, and other community partners
  • Deliver practice support services aligned with FPSC direction, practice needs, and community needs
  • Build shared understanding of issues and priorities among client groups
  • Lead planning and implementation of change programs across multiple stakeholders
  • Identify process enhancement opportunities and recommend strategically impactful changes
  • Establish monitoring and feedback expectations and review performance trends
  • Evaluate progress and involve peers and team members in analyzing performance
  • Improve efficiency through pilots and planned functional change initiatives
  • Adapt priorities and approaches to changing workloads and organizational goals
  • Analyze issues, patterns, trends, strengths, weaknesses, errors, successes, and failures
  • Develop short-term program strategies aligned with organizational priorities and values
  • Build and maintain relationships with professional groups and organizations
  • Facilitate meetings and group sessions to maximize participation
  • Provide constructive, respectful, and diplomatic partnership with client groups

Requirements

What you’ll need
  • A Bachelor's degree in Health Services, Health Information Management, Business Administration, or related field and 3-5 years of experience, or an equivalent combination of education and experience
  • Solid understanding of the BC health system and the operational context of primary care
  • Experience working directly in a family practice or in a supporting role is an asset
  • Demonstrated understanding and application of quality improvement methodologies, ideally in health care settings
  • Ability to understand linkages between workflow, structured data capture, meaningful reports, measures, and team/provider performance
  • Experience applying coaching approaches using questions and constructive feedback
  • Familiarity with one or more EMR systems, including registries, scheduling, visit flow, and registry-based recalls
  • Understanding of BC health care system, primary care operational models, financial incentives, and patient conditions and journeys
  • Experience and training in change management principles
  • Ability to facilitate small group sessions and prepare and run effective meetings
  • Demonstrated ability to work effectively across teams and develop trusting working relationships
  • Passion for improving primary care and supporting physicians
  • Service orientation and ability to exceed customer expectations
  • Ability to navigate varied personalities and adjust communication styles to emotional and cultural contexts
  • Travel within the region and to facilities and communities is required
  • Must have access to reliable transportation
