About the role

Key responsibilities & impact
  • Provide in-depth clinical pharmacy services to pediatric cell and gene therapy patients
  • Provide clinical pharmacy services to ambulatory pediatric hematology/oncology and pediatric transplant and cellular therapy patients
  • Participate in clinic and care management activities and oversee safe medication use
  • Complete and review medication reconciliation
  • Provide medication counseling
  • Prescribe and adjust medications as permitted through a collaborative practice agreement
  • Screen for drug interactions, therapeutic duplications, and omissions
  • Monitor therapeutic and adverse medication responses
  • Coordinate patient care, follow-up, and laboratory monitoring with outpatient and inpatient teams
  • Ensure continuity of care and medication access
  • Facilitate prescription fulfillment through Duke specialty and retail pharmacies
  • Coordinate with specialty, retail, external pharmacies, patient assistance, and prior authorization teams
  • Collaborate with infusion pharmacy staff on medication-related issues
  • Document care in patient records
  • Provide discharge counseling and education for select high-risk patients in person, virtually, or by telephone
  • Provide direct patient care and virtual services as directed by oncology, cell and gene therapy, and pharmacy leadership
  • Provide clinical input, education, and treatment-plan development oversight for pediatric cellular and gene therapies
  • Educate medical, nursing, and pharmacy staff on therapy management, prescribing, and monitoring
  • Serve as a pharmacy department resource
  • Participate in departmental and hospital committees and initiatives
  • Contribute to student and resident education
  • Provide educational lectures and/or participate in research
  • Work with peers toward division and departmental goals
  • Maintain personal and professional growth and contribute to the pharmacy team

Requirements

What you’ll need
  • Doctor of Pharmacy Degree OR BS Pharmacy Degree
  • With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required
  • With Doctor of Pharmacy degree, no experience is required
  • Three plus years of oncology clinical practice experience preferred
  • PGY2 oncology residency or PGY2 Pediatric residency preferred
  • Clinical Pharmacist Practitioner to be obtained within one year of hire
  • Licensed Pharmacist or eligible to practice pharmacy in North Carolina
  • Ability to provide clinical pharmacy services to pediatric oncology, hematology/oncology, transplant, cellular therapy, and cell and gene therapy patients
  • Ability to prescribe and adjust medications under a collaborative practice agreement
  • Ability to monitor medication efficacy, adverse responses, interactions, duplications, and omissions
  • Ability to coordinate laboratory monitoring, follow-up, medication access, prior authorizations, and prescription fulfillment
  • Ability to document patient care in medical records
  • Ability to educate medical, nursing, and pharmacy staff
  • Ability to participate in committees, initiatives, education, and research
