About the role

Key responsibilities & impact
  • Support the mental and physical health care of patients on an assigned patient caseload.
  • Screen and assess patients for common mental health and substance abuse disorders; facilitate patient engagement and follow-up care.
  • Provide patient education about mental health and substance abuse disorders and available treatment options.
  • Track treatment response and monitor patients in person or by telephone for clinical changes and treatment side effects or complications.
  • Support psychotropic medication management by consulting with the behavioral health consultant and communicating with the assigned primary care provider.
  • Provide brief behavioral interventions using evidence-based techniques, including behavioral activation, problem-solving treatment, and motivational interviewing.
  • Provide or facilitate referrals to evidence-based psychosocial treatments.
  • Participate in regular caseload consultation with psychiatric and medical providers and communicate treatment recommendations.
  • Track patient follow-up and clinical outcomes using a registry; document encounters and re-engage patients.
  • Document patient progress and treatment recommendations in the EHR and other required systems.
  • Facilitate treatment plan changes and referrals for specialized mental health, substance abuse, housing, and vocational services.
  • Develop relapse prevention self-management plans for patients nearing discharge from the caseload.
  • Complete daily assessments and productivity trackers using Excel, Word, calendars, and the EMR system.
  • Conduct primarily telephonic work with occasional in-person patient visits.

Requirements

What you’ll need
  • The Social Worker Care Manager will have a minimum of MSW
  • Preferred Licensure or provisionally licensed
  • Behavioral Health experience and knowledge
  • Must be licensed driver with an insured automobile in good working order
  • Ability to communicate effectively, both verbally and in writing.
  • Time Management skills
  • Comfortable utilizing Excel and Word and completing assessments and productivity trackers daily
  • Ability to manage the day effectively through calendars and the EMR system
  • Candidates must disclose any current or future need for employment-based immigration sponsorship, including OPT, STEM OPT, or visa sponsorship, before an offer of employment is extended.
