About the role

Key responsibilities & impact
  • Review daily remote patient monitoring readings, including blood pressure, blood glucose, weight, pulse, oxygen saturation, and other assigned health metrics
  • Monitor for missing readings, abnormal values, and concerning trends
  • Contact patients about abnormal or missed readings, symptoms, medication adherence, and participation barriers
  • Encourage consistent patient engagement and program participation
  • Follow escalation protocols and communicate concerning findings to providers or licensed clinical staff
  • Coordinate care with physicians, care managers, medical assistants, pharmacies, and other healthcare team members
  • Enroll eligible patients into the RPM program and educate them on devices, expectations, and participation requirements
  • Identify patients needing additional care management, provider follow-up, or community resources
  • Educate patients on device use, measurement techniques, medication adherence, nutrition, activity, symptom monitoring, and chronic disease management
  • Troubleshoot RPM devices, applications, and connected monitoring technology
  • Document patient interactions, readings, interventions, education, and escalations in the electronic health record
  • Verify patient consent, diagnoses, care plans, and device assignments
  • Support RPM device inventory, replacement, return, and tracking
  • Maintain compliance with HIPAA, payer requirements, and organizational policies
  • Meet productivity, documentation, and patient outreach expectations
  • Participate in team huddles, quality audits, training, and continuous improvement initiatives
  • Confirm abnormal readings, assess associated symptoms, follow clinical escalation pathways, notify licensed clinical personnel or providers, and direct patients to emergency services when indicated
  • Do not independently diagnose conditions, prescribe medications, or provide treatment outside the scope of credentials

Requirements

What you’ll need
  • High school diploma or GED required
  • Healthcare experience as a Medical Assistant, Pharmacy Technician, Certified Nursing Assistant, Care Manager, Health Navigator, or similar role preferred
  • Experience in primary care, chronic care management, patient outreach, population health, or remote patient monitoring preferred
  • Knowledge of chronic conditions such as hypertension, diabetes, COPD, heart failure, obesity, and cardiovascular disease
  • Strong computer skills and experience with electronic health records
  • Excellent communication, organizational, and time management skills
  • Ability to recognize abnormal readings and follow established escalation protocols
  • Current healthcare certification or license preferred
  • Experience with Athenahealth, ChronicCareIQ, 3CX, or similar healthcare platforms preferred
  • Knowledge of Medicare, Medicaid, value-based care, or RPM programs preferred
  • Background check required
