About the role

Key responsibilities & impact
  • Lead field clinical operations within an assigned area.
  • Lead, coach, and manage assigned field clinicians through one-on-one meetings, team huddles, performance reviews, and corrective coaching.
  • Translate Clinical Operations priorities, policies, and workflows into consistent area execution.
  • Reinforce scheduling, documentation, communication, supply, mileage, safety, and escalation processes.
  • Support clinician onboarding, field integration, competency completion, and readiness to practice independently.
  • Monitor patient demand, staffing capacity, clinician schedules, and geographic coverage; address operational barriers and escalate recruiting or coverage needs.
  • Use area scorecards to identify trends, coach performance, implement improvement plans, and provide updates on patient risks, staffing, results, and action plans.
  • Monitor active patients for scheduling gaps and required follow-up and dressing-change visits.
  • Oversee rescheduling after cancellations, missed visits, hospitalizations, and incomplete visits; manage holds and post-discharge follow-up.
  • Identify and address risks for gaps in care, disengagement, avoidable discharge, or failure to meet ordered care frequency.
  • Ensure required in-home initial visits and appropriate NP follow-up, dressing changes, and other visits are assigned to the correct clinical role.
  • Partner with Intake, Patient Care Coordination, Member Engagement, Medical Leadership, Education, and other teams to strengthen handoffs and resolve barriers.
  • Monitor timely and complete documentation, including visit-note finalization, medical necessity, procedure, supply, and patient communication workflows.
  • Serve as the primary operational escalation point for field clinicians.
  • Escalate changes in condition, patient safety concerns, scope-of-practice questions, and medical decision-making issues to the appropriate Medical Director.
  • Support audits, corrective action plans, payor requirements, regulatory readiness, and resolution of patient complaints and field service concerns.

Requirements

What you’ll need
  • Bachelor’s degree in nursing, healthcare administration, business administration, or a related field, or equivalent relevant experience.
  • Active, unrestricted Registered Nurse license and a bachelor’s degree in nursing.
  • At least five years of experience in healthcare operations, clinical operations, field leadership, home-based care, or a related setting.
  • At least two years of direct people-management experience, preferably leading licensed clinicians.
  • Experience managing performance through operational and clinical quality metrics and coordinating staffing, scheduling, geographic coverage, and patient-service needs.
  • Working knowledge of HIPAA, healthcare documentation, patient safety, and escalation requirements.
  • Strong organizational and communication skills, with the ability to manage competing priorities and address performance concerns directly and supportively.
  • Ability to distinguish operational issues from concerns requiring Medical Leadership involvement and to build structure in a growing, multi-state organization.
  • Ability to travel within the assigned area and participate in field visits or ride-alongs as needed.
  • Preferred: Experience in wound care, home health, home-based primary or specialty care, or another mobile clinical delivery model.
  • Preferred: Experience leading Nurse Practitioners, Registered Nurses, and Licensed Practical Nurses.
  • Preferred: Knowledge of Medicare, Medicare Advantage, commercial payors, and value-based care models.
  • Preferred: Experience with clinician onboarding, competency validation, audits, corrective action, and regulatory readiness.
  • Preferred: Experience supporting multi-state operations and rapidly growing markets.
  • Preferred: Experience with BOP or a similar electronic health record, care-management, scheduling, or task-management platform.
  • Preferred: Wound care certification or meaningful provider-level or nursing wound care experience.
