About the role

Key responsibilities & impact
  • Provide outpatient orthopedic physical therapy care for an adolescent to adult caseload from 11 years old and up
  • Communicate evaluation results, treatment plans, and goals to patients and/or parents/caregivers
  • Provide home program activities and engage patients and/or caregivers in treatment sessions
  • Complete accurate, timely, and thorough documentation in accordance with department and insurance requirements
  • Consult with physicians, allied health professionals, and other team members
  • Participate in case conferencing and program planning meetings
  • Facilitate training and presentations for staff, patients, and parents/caregivers when applicable
  • Evaluate patients referred for physical therapy and consultation
  • Perform evaluations according to patient needs, physician recommendations, and program protocols
  • Communicate with Referral, Case Management, Care Coordination, and Scheduling staff
  • Make and follow through with referrals when appropriate
  • Provide oversight and supervision of Physical Therapy Assistants as necessary
  • Determine when the Physical Therapist must personally render interventions versus utilizing a Physical Therapy Assistant
  • Provide treatment in accordance with established standards of care and scope of practice
  • Design treatment plans and monitor progress through observations and assessments
  • Conduct re-evaluations when indicated
  • Perform other duties as assigned

Requirements

What you’ll need
  • California State licensure as a Physical Therapist
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
  • Trauma Life Support, or Advance Resuscitation Training
  • At least 2 years’ experience as a Physical Therapist preferred
  • Traveling between sites and other locations is occasionally required
  • Valid California driver’s license
  • Automobile and proof of minimum levels of car insurance as required under California law
  • Acceptable driving record
  • Demonstrated commitment to community medicine and providing culturally competent care to the medically underserved
