Perform comprehensive occupational therapy evaluations, treatment, and related follow-up services, including discharge planning
Document patient assessments and pertinent information in a timely manner

Provide occupational therapy, including selection and use of adaptive equipment
Collaborate with patients and other healthcare personnel or outside agencies regarding coordination of care

Requirements

Bachelor's Degree in Occupational Therapy - Required
1 year Occupational Therapy Experience - Required
1 year Home Health Experience - Preferred
Occupational Therapist (OT) State Licensing Board - Required
Cardiopulmonary Resuscitation (CPR), American Heart Association - Required
Driver's License - Required
Auto Insurance - Required
Travel required: up to 75%
