About the role
Key responsibilities & impact- Make initial nursing evaluation visits, communicate with physicians, and determine plans of care
- Practice nursing according to Franciscan Alliance standards, state nursing board standards, nurse practice acts, and governing regulations
- Provide emotional, psychological, and spiritual support to patients, families, and caregivers
- Collaborate with patients, families, and multidisciplinary teams to optimize outcomes and patient experience
- Assess and analyze comprehensive patient data to determine nursing diagnoses, needs, and plans of care
- Develop, implement, continuously review, and revise individualized plans of care
- Coordinate care delivery and interventions to improve patient outcomes in a safe, respectful, and ethical environment
- Evaluate patient progress toward outcomes with patients and families
- Communicate assessment changes to providers and coordinate multidisciplinary communication for safe transitions and continuity of care
- Promote patient independence through goal-setting, education about conditions and medications, and self-care instruction
- Maintain clean and safe work environments according to infection-control policies
- Complete equipment preventive maintenance or arrange repairs
- Supervise clinical care provided by LPNs and Home Health Aides
- Participate in an on-call rotation and provide care outside regular business hours
Requirements
What you’ll need- Associate's Degree in Nursing required
- Bachelor's Degree in Nursing preferred
- 1 year acute care experience required
- 2 years home health experience preferred
- 2 years long-term care experience preferred
- Registered Nurse (RN) state licensing board credential required
- Basic Life Support (BLS) certification from the American Heart Association required
- Driver's License required
- Auto Insurance required
- Availability for weekend shifts: Saturday and Sunday, 7am to 7pm, plus either Friday or Monday, 7am to 7pm
- Up to 75% travel required
