About the role
Key responsibilities & impact- Provide remote patient care and support through telehealth platforms
- Conduct virtual patient assessments, including reviewing medical histories and current symptoms
- Accurately assess and document patient conditions, histories, and care interactions
- Educate patients and families on medical conditions, medication adherence, and self-care
- Collaborate with healthcare providers to develop and implement individualized care plans

- Conduct remote outreach to Medicare members
- Support member compliance with a new Stars measure and confirm successful emergency department discharge
- Potentially close additional HEDIS measures
Requirements
What you’ll need- Bachelor of Science in Nursing (BSN) degree
- Active, non-restricted compact state nursing license with primary state of licensure in one of the listed compact jurisdictions
- 2+ years of experience as a Registered Nurse
- 1–2 years of experience in a remote healthcare or telehealth setting
- Strong computer skills and familiarity with electronic medical record (EMR) systems
- Ability to communicate clearly and effectively in both verbal and written forms
- Demonstrated ability to follow standard operating procedures and adapt in a fast-paced environment
- Strong customer service and interpersonal skills, especially in high-volume or high-stress scenarios
- Ability to work independently and as part of a collaborative care team
- Experience working in a startup environment (preferred)
- Applicants must be authorized to work for any employer in the U.S.; visa sponsorship is unavailable
- Ability to work at a computer terminal for extended periods and perform the stated physical requirements
