About the role

Key responsibilities & impact
  • Manage Journey patient census and referrals for the assigned area
  • Review patient charts to determine eligibility and appropriateness for the Hospice benefit
  • Make initial and follow-up phone calls to Journey referrals and assess patient decline
  • Conduct patient visits and discuss the hospice benefit with patients and families
  • Maintain Journey patient charts and document visits and contacts
  • Build relationships and communicate effectively with Clinicians
  • Communicate and collaborate with the Palliative team and NP
  • Attend monthly Interdisciplinary team meetings
  • Create and maintain Journey charts for each Journey patient
  • Meet or exceed expectations for weekly patient visits, weekly patient phone calls, and monthly transitions to hospice care
  • Educate clinicians on the philosophy, goals, eligibility, and benefits of Hospice care
  • Assist Hospice and Home Health teams with transitions to Hospice, including visits, calls, paperwork, and other tasks
  • Comply with the Company's Core Values and Core Competencies
  • Perform other duties as assigned

Requirements

What you’ll need
  • High school diploma/GED
  • Prior home healthcare or related work experience in the healthcare industry preferred
  • Understanding of the hospice philosophy preferred
  • Must have and maintain in good standing a professional license, certificate, or registration, as applicable
  • Successful completion of a drug screen prior to employment
  • Legally authorized to work in the United States
  • Ability to perform the essential duties satisfactorily
  • Ability to meet the stated physical demands, including frequently lifting and/or moving up to 20 pounds
  • Ability to travel by automobile and work in changing weather conditions
