About the role
Key responsibilities & impact- Work in alignment with the organization’s values, maintaining a results-oriented approach to care while respecting each patient’s individuality
- Record all information relevant and essential to the care process in the patient’s medical record and keep it up to date
- Represent the organization by providing care based on scientific evidence and best practices
- Provide comprehensive, compassionate patient care, building trusted relationships and addressing biopsychosocial needs
- Ensure continuity of medical care through implementation of the established Care Plan
- Manage the consultation schedule appropriately, fulfilling the contracted working hours and number of appointments
- Provide healthcare in the home setting, following the principles of the Buurtzorg model
Requirements
What you’ll need- Medical degree with residency training or postgraduate qualification in Geriatrics and/or Family and Community Medicine
- Active registration with the Regional Council of Medicine (CRM)
- Previous experience in home healthcare or community health is a plus
- Passion for promoting patient self-care and autonomy through a person-centered approach
- Strong communication and empathy skills
- Ability to work collaboratively within multidisciplinary teams
- Proactive approach to identifying solutions that promote patients’ quality of life and functional independence
