About the role

Key responsibilities & impact

Support people living with dementia and those who care for them as a trusted Dementia Support Coach.
Work in diverse care settings through the Hospital to Aged Care Dementia Support Program (HACDSP).
Deliver hands-on, evidence-based dementia support and advice to clients, carers and care staff.
Work collaboratively with peers to develop the skills, expertise and applied knowledge of aged care staff and carers.
Support implementation of recommended Dementia Support Australia plans using modelling and coaching techniques.
Seek guidance from Team Leaders and Dementia Consultants as required.
Manage and coordinate brokerage items with clients and carers to improve quality-of-life outcomes.
Act as a key liaison and trusted partner for staff and carers, building rapport and confidence in dementia care practice.

Requirements

What you’ll need

Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
The ability to work both independently and as part of a team.
Demonstrated commitment to excellence in customer service principles, including conflict resolution to ensure the consistent provision of exceptional service to internal and external customers
A current driver’s licence and safe driving record
Willingness to travel and work intrastate and interstate to support the delivery of Dementia Support Australia services.
Qualification in relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable
