About the role

Key responsibilities & impact
  • Provide superior patient support and customer service through proactive intervention, communication, patient advocacy, emotional support, and educational support
  • Coordinate seamless care delivery for patients with amputations or limb difference under the general direction of the ACM/CM or designee
  • Provide patient consultation, comfort, and guidance from initial referral through surgery, recovery, treatment, rehabilitation, and device fitting
  • Coordinate and administer prosthetic and orthotic patient care
  • Build relationships with healthcare providers and foster new opportunities to serve more patients
  • Manage daily communication with patients, care team members, and referral partners
  • Contact new patients before or after initial consultation or AmpuShield fitting and enter them into Hanger Clinic’s patient management and tracking system
  • Support patients and families before and after surgery with information, resources, advice, and counseling on prosthetic/orthotic options
  • Coordinate AMPOWER visits
  • Build and maintain referral relationships for Hanger Clinic
  • Assist with obtaining clinical documentation for insurance coverage
  • Conduct follow-up communications and appointments to maintain engagement and continuity of care
  • Provide updates on surgeon follow-up visits, including wound descriptions, photos, care-plan details, and modifications
  • Complete hospital, skilled nursing facility, or rehabilitation facility visits as needed
  • Identify and follow up on referrals to ensure continuity of care and patient/family needs
  • Serve as contact for patients, families, physicians, outside agencies, staff, and the general public regarding clinic services
  • Liaise between patients and clinic areas/ancillary services for pre-operative and post-operative planning, preparation, and care coordination through definitive device delivery
  • Use CCC clinical communication and tracking tools to maintain communication with patients and health professionals
  • Assist patients with upcoming visits and fittings, potentially requiring ABC Certified Orthotic Fitter credential

Requirements

What you’ll need
  • Minimum 2 years of experience in a patient care environment
  • BA/BS in Health related field or equivalent work experience
  • Ability to be credentialed in major hospital systems
  • Experience providing a high level of customer service in a fast paced environment, including handling confidential data with discretion
  • Must have, or be eligible to obtain, a valid driver’s license and driving record within Hanger’s Motor Vehicle Safety Policy and Procedures
  • Strong interpersonal, oral and written communication skills
  • Strong customer service skills
  • Empathetic, kind, engaging personality
  • Non revoked, valid passenger vehicle driver’s license
  • Excellent organizational skills
  • Understanding of Insurance
  • Time management and ability to maintain a schedule
  • Experience interfacing with physicians, nurses, physical therapists, occupational therapists and clinical support personnel
  • Ability/willingness to work with handicapped individuals
  • SharePoint Post-Acute CRM versed
  • Understanding of medical reimbursement and terminology and Hanger Clinic front office duties preferred
  • Ability to use a computer, including Windows-based software and e-mail
  • Ability to use all necessary office equipment
  • Preferred: ATC, EMT, MA, Surgical Tech, LVN, PTA, Physician Extenders, OTA, Social Work or like certification or equivalent experience
  • Preferred: Experience with progressive responsibilities in health care facility administrative support functions such as insurance processing coordinator, patient care coordinator, MA, or related roles
