About the role
Key responsibilities & impact- Respond to inquiries, coordinate appropriate services, and conduct in-person or virtual clinical assessments to develop service plans
- Provide empathetic support and crisis intervention
- Coordinate communication among service providers and formal and informal supports
- Build referral networks and maintain resource directories
- Strengthen relationships with community and program partners
- Support program implementation, outreach, promotion, awareness activities, and community events
- Contribute to evaluation tools and processes
- Collect, document, and report program data and outcomes
- Obtain consent and safeguard personal information in accordance with PHIA, FIPPA, and professional standards
- Communicate concerns promptly and represent the Centre professionally
- Comply with organizational, safety, legislative, regulatory, and accreditation requirements
Requirements
What you’ll need- A BSW, OT, PT, BSc PN, RPN, BN, SLP, RD, or related health or human services degree
- Membership in good standing in the applicable professional association or regulatory body
- Proficiency in both of Canada’s official languages, orally and in writing
- Experience in mental health or clinical coordination
- Knowledge of Indigenous history and its impacts
- Commitment to psychosocial rehabilitation and recovery, primary health care, trauma-informed practice, and person-centred planning
- Excellent communication, conflict-resolution, relationship-building, critical-thinking, and organizational skills
- Ability to support vulnerable populations, manage competing priorities, and work effectively in a fast-paced environment
- Proficiency in Outlook and Word
- Ability to perform the physical duties of the position in varied weather and environmental conditions
- Subject to a Criminal Record Check including Vulnerable Sector Search, Adult Abuse Registry Check, and Child Abuse Registry Check; successful candidate responsible for service charges
