About the role

Key responsibilities & impact

Provide education about patients’ diagnoses, test results, and treatment plans
Collaborate with cardiologists, PCPs, and other healthcare professionals to coordinate patient care
Discuss complex cases and participate in multidisciplinary care teams
Use clinical decision-making to escalate concerns to the care team
Maintain accurate patient records, including interactions, medical histories, assessments, treatment plans, progress notes, and discharge summaries
Schedule and conduct follow-up virtual visits
Advocate for patients’ needs and preferences
Adhere to legal, ethical, confidentiality, and telemedicine requirements
Triage incoming patient requests and prioritize the patient panel based on need and client SLAs
Pursue continuing education and maintain relevant certifications
Participate in quality assurance programs to improve cardiology care
Support the virtual cardiology team and coordinate care in a fast-paced remote environment

Requirements

What you’ll need

Must hold a current and unrestricted Registered Nurse (RN) license (Compact)
At least 3+ years of experience in cardiology
Telemedicine experience is a bonus
Excellent communication and interpersonal skills, especially in a virtual environment
Ability to educate patients and collaborate with a multidisciplinary healthcare team
Ability to work independently and make clinical decisions under appropriate supervision
Proficiency in telehealth platforms and electronic health records
Commitment to patient-centered care and empathy for patients with chronic cardiology conditions
Compassion and empathy when working with patients and their families
