About the role

Key responsibilities & impact
  • Review client activity, including drug test results, system logins, lesson completion rates, and appointment attendance and adherence
  • Monitor client progress against individualized treatment plans
  • Identify deviations from treatment plans and collaborate with the clinical team on interventions
  • Identify and proactively address barriers to client engagement
  • Verify client insurance coverage and eligibility for treatment services
  • Coordinate with Justice Liaisons and facility managers to schedule and manage in-person and telehealth appointments at correctional facilities
  • Schedule and reschedule client appointments with providers
  • Support the clinical team with client follow-up
  • Help clients identify and access community resources and referrals, including housing, transportation, and employment
  • Coordinate referrals with external agencies and providers
  • Troubleshoot technical issues related to the proprietary technology platform
  • Guide clients in navigating the platform and accessing program resources
  • Respond to client inquiries and concerns
  • Provide ongoing support and encouragement throughout clients’ treatment journeys

Requirements

What you’ll need
  • Strong organizational and relationship-building skills
  • Ability to coordinate effectively across health plan Justice Liaisons and facility managers
  • Outstanding communication and interpersonal skills
  • Ability to engage with individuals from diverse backgrounds in a judgment-free and culturally sensitive manner
  • Exceptional organizational and time management skills
  • Ability to conduct concise yet supportive phone conversations
  • Understanding of the community served, the justice system, and community connectedness
  • Strong understanding of the healthcare system, including medical insurance
  • Ability and willingness to provide emotional support, encouragement, and motivation to clients
  • Written and oral fluency in English and Spanish preferred
  • Strong computer skills
  • Prior experience using an EHR/EMR strongly preferred
  • Strong confidentiality due to sensitive materials and information
  • Ability to work independently, be self-directed and flexible, and perform functions with minimal supervision
  • Reliable transportation
  • Willingness to travel on site to facilities in Springer, Los Lunas, and Grants
  • High school graduate or equivalent required
  • Associate degree preferred
  • 1–3 years of experience working in a behavioral health setting preferred
