About the role

Key responsibilities & impact
  • Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning
  • Diagnose health conditions and create individualized treatment plans
  • Prescribe and manage medications, monitor for side effects, and adjust treatment as needed
  • Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
  • Serve as a clinical consultant on complex or high-need cases
  • Educate clients and families on treatment plans, medications, and recovery support strategies
  • Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
  • Participate in team meetings, professional development, and continuous training
  • Provide telehealth-based assessments, medication management, and care coordination
  • Work closely with hybrid Care Navigator, Case Manager, and Peer Recovery Specialist teams
  • Support youth clients aged 12-17 on their recovery journey

Requirements

What you’ll need
  • Active and unrestricted NP license in Tennessee
  • Active DEA license (and CDS where applicable)
  • Several years of experience in behavioral health, addiction medicine, or psychiatry
  • Experienced in working with youth populations and their caregivers, specifically those in the 12-17 age range
  • Experienced in addiction medicine or psychiatry
  • Board certification in specialty preferred
  • Strong documentation skills and proficiency in EHR platforms
  • Clear and confident virtual communication
  • Ability to collaborate in integrated and team-based care models
  • Organized, flexible, and capable of navigating complex systems
  • Adaptable to evolving schedule needs
  • Commitment to person-centered care
  • Must support clients aged 12-17 experiencing behavioral health and/or SUD needs
  • Tennessee licensure required; additional state licenses are a plus
