About the role

Key responsibilities & impact
  • Welcome agency guests, including program participants, volunteers, and staff
  • Conduct participant orientation and intake
  • Maintain a caseload and provide case management and direct services
  • Develop and implement individualized housing plans
  • Provide community-based case management services
  • Collaborate with community partners, the EIH team, and external case managers
  • Complete safety planning, risk assessments, and mandated reporting
  • Communicate participant successes and challenges to case management teams
  • Maintain accurate case management records and grant-compliance documentation
  • Provide support using Critical Time Intervention and strength-based approaches
  • Meet participants according to their needs
  • Participate in property management meetings and case conferencing
  • Transition residents to the next appropriate level of care
  • Implement agency and program policies and procedures
  • Complete incident reports, behavior contracts, and gross-violation reports
  • Attend job-related, program, and agency-wide meetings
  • Participate in learning and skill-development opportunities
  • Document case management activity and progress notes in HMIS within three business days
  • Conduct ongoing reassessments and document them in HMIS
  • Analyze and improve program services and align them with industry best practices
  • Assist with office-related and other assigned duties

Requirements

What you’ll need
  • Minimum two years’ experience in social services, unless the applicant has a bachelor’s degree in psychology, counseling, mental health, human services, or a related field
  • Lived experience or understanding of and sensitivity to the needs of the unhoused population
  • Fluent in Microsoft Office Suite
  • Ability to apply Housing First and Harm Reduction principles
  • Ability to work with unhoused and low-income populations
  • Ability to effectively de-escalate and manage individuals who may be experiencing serious mental illness or intoxication
  • Strong communication and active listening skills
  • Good problem-solving skills
  • Bilingual capabilities are a plus
  • Candidates with previous experience serving unhoused and low-income populations are preferred
