About the role
Key responsibilities & impact- Guides care coordination for high risk acute care patients.
- Facilitates discharge plans for patients with medically complex post-acute care needs.
- Analyzes medical records to determine legitimacy of admission, treatment, and length of stay in acute settings.
- Advocates for patients and families and liaises with key team members.
- Collaborates with physicians, patients, providers, and stakeholders to coordinate health services and referrals.
- Determines appropriate level of care, resource utilization, and length of stay using electronic admission and discharge criteria and physician input.
- Communicates healthcare-related information verbally and in writing to coordinate care and services.
- Manages transitions of care and reevaluates transition needs daily.
- Coordinates transitions to the next level of case management or care coordination services.
- Performs concurrent reviews of patient treatment plans according to hospital policies and third-party reimbursement systems.
- Maintains department logs, databases, statistics, and utilization review documents.
- Performs other duties as assigned.
Requirements
What you’ll need- Associates in Nursing Required
- Registered Nurse (RN) - License, State Licensure And/Or Compact State Licensure Required
- 3 years, clinical experience in a hospital setting Required
- Bachelor's in Nursing or healthcare related field Preferred
- 5 years, as an RN in Case Management Preferred
- 7 years, as a Registered Nurse (RN) Preferred
- Experience in an outpatient clinical setting working with PCP, Home Health or Rehab Preferred
- Certified Case Manager - Certification, in Case Management Preferred
