About the role

Key responsibilities & impact
  • Manage an assigned caseload and coordinate care across the health care continuum
  • Collaborate with patients, caregivers, healthcare providers, and external partners to support safe transitions of care
  • Prepare patients for discharge, including supportive care, end-of-life decisions, community resources, goal setting, and long-term planning
  • Conduct interviews and arrange post-acute interventions, including home visits and behavioral health service coordination
  • Assess readmission risk and outpatient barriers to care
  • Initiate and assist patients with advance directives
  • Facilitate communication of clinical information and care plans among hospitalists, specialists, PCPs, and other providers
  • Communicate financial obligations and discharge information to patients, families, MPOA, and payers
  • Obtain authorization for services and complete hand-offs to appropriate case management teams
  • Promote integrated care collaboration among healthcare team members, patients, and families
  • Maintain case management metrics and manage the caseload
  • Advocate for patients and monitor care quality using standards of care and evidence-based practice guidelines
  • Perform other related duties as assigned or requested

Requirements

What you’ll need
  • Master's in Social Work required
  • 1 year as a Licensed Social Worker, and/or successful completion of health related field placement in Master's level Social Work Program required
  • Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) license required
  • 2 years in case management preferred
  • 3 years as a Licensed Social Worker preferred
  • Experience in management of behavioral health patients preferred
  • Certified Case Manager certification preferred
