About the role

Key responsibilities & impact

Conduct outreach, scheduling, and care management for assigned patients
Establish relationships with physicians, clinical staff, patients, caregivers, payers, and community resources
Use motivational interviewing to build trust and support health-management goals
Review patient lists and disease registries to identify high- and emerging-risk patients
Evaluate patients for Chronic Care Management enrollment
Perform Medicare Wellness Visits
Coordinate medical treatment, complete assessments, develop care plans, monitor medication compliance, and communicate with primary care providers
Complete Transition of Care outreach and schedule appropriate follow-up
Document outreach efforts and outcomes for gap closure
Monitor medication-adherence gap reports and provide education and interventions
Connect eligible patients with Medication Therapy Management programs
Coordinate Remote Patient Monitoring escalations and referrals
Identify barriers to preventive screenings, wellness visits, and follow-up care
Connect patients with community resources and maintain resource databases
Maintain accurate records and report to managers and directors
Contribute to closing gaps in care
Participate in meetings and collaborate with internal and external stakeholders
Perform other duties as assigned

Requirements

What you’ll need

RN, LPN, or CMA licensure required
Experience with Patient Navigation and ACO preferred
Demonstrated verbal and written communication skills
Conflict resolution skills
Interdisciplinary collaboration skills
Care management skills
Creative problem-solving skills
Critical thinking skills
Knowledge of community resources
Current BLS certification or ability to obtain within 30 days of hire
Monday–Thursday, 7 a.m.–4 p.m. availability
