About the role
Key responsibilities & impact- Assess, plan, implement, and evaluate patient care using the nursing process
- Facilitate advance care planning conversations with patients and families
- Assist with completion of advance directives and state-specific ACP documents
- Facilitate POLST/POST/MOST discussions in accordance with policies and regulations
- Educate patients, caregivers, and clinical teams on goals of care, surrogate decision makers, treatment preferences, ACP workflows, documentation standards, and communication skills
- Support centralized ACP referrals and patient outreach
- Conduct ACP facilitator training and competency validation
- Review ACP documentation quality and improve documentation accuracy with HIM and clinical teams
- Participate in quality improvement, data collection, reporting, and program evaluation
- Serve as an ACP consultant and resource across the organization
- Collaborate with palliative care leaders on ACP strategy, implementation, and program growth
- Work with primary care, specialty care, palliative care, care management, social work, and community partners
- Provide virtual and in-person patient and caregiver interactions across ambulatory and inpatient settings
Requirements
What you’ll need- Current Registered Nurse license
- Minimum three years of RN experience, including acute, care management, hospice, and palliative care
- Experience facilitating goals-of-care or serious illness conversations
- Strong communication, presentation, and education skills
- Ability to work independently and collaboratively across multidisciplinary teams
- Experience with electronic medical record documentation
- Ability to read patient information and assess patient needs
- Ability to communicate with patients, visitors, providers, caregivers, and care team members
- Ability to operate medical equipment and computers for documentation
- Ability to lift, move, and transfer patients and transport supplies and equipment
