About the role

Key responsibilities & impact
  • Coordinate care for transplant patients, living donors, and their families
  • Facilitate patients' and families' movement through transplantation and living donation processes through education, support, and service coordination
  • Manage clinical aspects of the transplant program, including education, patient care coordination, organ acquisition, research, data management, quality assessment, and program development
  • Manage patient care from referral through post-operative management, discharge planning, and long-term follow-up
  • Liaise among transplant physicians, referring physicians, transplant recipients, families, living donors, and outside agencies
  • Make independent care and treatment decisions within established protocols
  • Provide on-call coverage and problem-solve during stressful situations
  • Facilitate patient entry into the transplant program
  • Coordinate medical, surgical, financial, psychosocial, nutritional, and pharmaceutical evaluations and ongoing care
  • Participate in care planning and selection conferences
  • List transplant patients and register living donors in a timely manner
  • Maintain the UNOS waiting list according to policies, regulations, and national guidelines
  • Coordinate pre-operative, inpatient, discharge, post-discharge, and ongoing case management
  • Communicate patient needs to multidisciplinary transplant team members
  • Provide age-specific care and intervention
  • Assess, plan, and advocate for patients and families across the care continuum
  • Document care accurately and promptly
  • Use critical thinking and involve patients, families, physicians, ancillary providers, and multidisciplinary teams in care planning
  • Facilitate organ acquisition and transplantation through on-call responsibilities
  • Collect and analyze data for physicians, leadership, regulatory agencies, and research protocols
  • Participate in quality monitoring and process improvement
  • Follow transplantation and living-donation regulations, policies, procedures, and guidelines
  • Assess and address education needs related to transplantation, organ procurement, and organ donation
  • Participate in community and professional education promoting organ donation and transplantation

Requirements

What you’ll need
  • RN license to practice in state(s) where care is provided
  • Basic Life Support Certification (BLS) for healthcare providers
  • Unit-specific certification as required (BLS, ACLS, PALS, etc.)
  • RNs hired or promoted into this role must have or obtain a BSN within three years of hire or promotion
  • Two years of acute care RN experience
  • Available to perform after-hour and weekend call rotation
  • Effective verbal and written communication skills
  • Effective interpersonal problem solving, prioritization, and independent judgment skills in a fast-paced, challenging work environment
  • Experience working well under pressure
  • Bachelor's degree in Nursing (BSN) from an accredited institution; degree will be verified
  • Clinical Certification of Transplant Coordinators (CCTC)
  • Three years of acute care or charge nurse experience
  • Two years of organ transplant coordination experience
  • Ability to see and read information, labels, monitors, equipment, and supplies
  • Ability to verbally communicate and hear and understand spoken information, alarms, needs, and issues quickly and accurately, especially during emergencies
  • Manual dexterity to manipulate complex and delicate equipment with precision and accuracy
  • Frequent computer use and typing for documenting patient care and accessing information
