About the role
Key responsibilities & impact- Schedule appointments with external network specialists for consultations and procedures ordered by PACE Primary Care Physicians
- Prioritize appointment scheduling based on Expected By Date and clinical urgency
- Collaborate with clinical teams to manage participant flow and timely access to care
- Document appointment data and updates accurately in Epic EMR
- Liaise between participants, caregivers, providers, and external specialist offices
- Notify participants and families of appointment details
- Coordinate complex medical appointments, referrals, and specialty care services
- Obtain and track consultation notes and diagnostic testing results
- Manage clinic-based scheduling workflows and participant care coordination
- Perform front-office support, including participant check-in and appointment management
- Prepare and coordinate authorizations for specialty referrals and services
- Document scheduling activities and participant interactions in the EMR
- Collaborate with the Interdisciplinary Team, Transportation Department, Adult Day Health Center, and clinic staff
- Participate in IDT meetings as appropriate
- Coordinate schedule changes, transportation adjustments, medically necessary escorts, and coverage needs
- Support internal audits, quality reviews, and process improvement initiatives
- Perform other duties assigned by the Regional Scheduling Manager
Requirements
What you’ll need- 1+ year of administrative, clerical, and medical administrative experience including filing, record keeping, data entry, and customer service
- 1+ year of scheduling experience
- Recent medical scheduling experience within the last 1–2 years of employment
- 3–5 years of customer service experience
- Remote work experience
- Medical terminology knowledge
- EMR experience
- Ability to perform each essential duty satisfactorily
- Preferred: clinic health care experience in a geriatrics setting
- Preferred: experience as a Nursing Assistant in home care, long-term care, daycare, or group home
- Preferred: experience using Epic EMR
