About the role

Key responsibilities & impact
  • Schedule and confirm patient appointments, follow-ups, and referrals
  • Notify communities/facilities and providers of scheduled visit dates
  • Request consultations, visit schedules, patient sign-up lists, and missing documentation
  • Organize and maintain monthly provider calendars in Outlook
  • Create visit templates and reports using scheduling software
  • Develop and maintain relationships with community/facility decision-makers
  • Identify account concerns and growth opportunities
  • Maintain updated account information in the CRM
  • Collaborate with practitioners and internal teams to ensure seamless patient care and customer satisfaction
  • Coordinate with patients, Power of Attorneys, and facilities
  • Monitor insurance approvals, outstanding referrals, authorization requests, rescheduling queues, and fax folders
  • Report the number of patients seen at each facility
  • Respond to voicemails in a timely manner
  • Format monthly reports into flyers for distribution
  • Complete tasks assigned in Planner

Requirements

What you’ll need
  • High School Diploma (or equivalent)
  • Prior healthcare experience, including patient care coordination or patient scheduling
  • Strong understanding of insurance eligibility using Availity and payer-specific platforms
  • Friendly, clear, and professional phone manners
  • Ability to handle multiple tasks accurately in a fast-paced environment
  • Comfortable navigating medical software and EHR systems
  • Compassionate mindset and genuine desire to prioritize patient care
  • High-speed internet access
