About the role
Key responsibilities & impact- Assist with coordinating post-hospital care and service arrangements under the direction of the Manager of Case Management and Case Manager RN
- Coordinate homecare, Durable Medical Equipment (DME), transportation, infusion services, other post-acute services, and authorizations for identified patients
- Interact with patients and families at the bedside and over the phone
- Deliver and obtain patient rights documentation
- Interact with co-workers, visitors, and other staff consistent with Jefferson's values
- Multitask and prioritize work in a fast-paced environment
Requirements
What you’ll need- High School Diploma required
- Bachelor's Degree preferred
- Experience working in a hospital setting or in a patient-facing role preferred
- Experience obtaining post-acute insurance authorizations using insurance company portals
- Computer literacy
- Strong knowledge of medical terminology
- Ability to multitask and prioritize work
- Part-time availability for 20 hours per week, including every Saturday and Sunday plus an additional 4-hour weekday shift
