About the role

Key responsibilities & impact
  • Facilitate safe, timely, and cost-effective transitions of care
  • Coordinate post-acute services and appointment needs for patients on the assigned unit
  • Schedule and link physician, laboratory, procedural, and financial appointments to appropriate orders and payor requirements
  • Communicate with third-party payers to secure authorization for inpatient and post-acute services
  • Assess psychosocial and environmental factors affecting recovery
  • Provide education and referrals to community resources
  • Support patients and families during transitions from hospital to home or another level of care
  • Arrange home health, DME, transportation, infusion services, specialty population services, and community-based resources
  • Obtain payer authorization and submit clinical information
  • Document discharge planning activities, authorizations, communications, and service arrangements in the medical record
  • Review and complete Medicare Notices of Hospital Discharge
  • Liaise with patients, families, healthcare providers, payers, and community agencies
  • Monitor and assist with intra-facility and acute-care hospital transfers
  • Coordinate transportation, authorization, and repatriation of medically stable patients
  • Communicate, escalate, and prioritize transportation needs with care management and the transfer center
  • Complete required regulatory documentation and admission assessments under supervision
  • Float among Washington Township, Cherry Hill, and Stratford locations

Requirements

What you’ll need
  • Bachelor’s Degree in healthcare related field
  • 2 years of experience in healthcare related field or less than 1 year as a new graduate
  • Ability to work constructively and collaboratively with a variety of individuals and groups
  • Ability to manage multiple concurrent objectives, projects, groups, or activities and prioritize time effectively
  • Computer and EHR literacy
  • Knowledge of patient appointment and procedure scheduling rules
  • Knowledge of insurance, Medicare, and Medicaid
  • Knowledge of HIPAA
  • Knowledge of medical terminology
  • Preferred: Bachelor’s Degree in social work
  • Preferred: Knowledge of discharge planning regulations, patient choice, and insurance authorization
  • Preferred: Familiarity with electronic health records and documentation
  • Preferred: Familiarity with medical terminology
  • Ability to lift and carry 40–50 lbs.
  • Frequent to continuous standing and walking
  • Ability to perform role-specific activities including kneeling, bending, squatting, and CPR as required
