About the role

Key responsibilities & impact
  • Provide patient- and family-centered nursing care using the nursing process
  • Address patients’ physical, psychological, emotional, and social needs
  • Collaborate with providers and support staff as part of the ambulatory care clinical team
  • Communicate respectfully with patients about individualized needs, priorities, cultural beliefs, and values
  • Integrate evidence-based practice, clinical expertise, patient preferences, and family/designated care partner preferences
  • Collaborate with primary care providers on patient care and management
  • Support communication between primary care providers and specialists
  • Implement clinical care pathways for common primary care conditions
  • Participate in quality improvement opportunities
  • Deliver ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication
  • Educate patients and families on preventive care, chronic disease management, medication management, and common illnesses
  • Document patient interactions and clinical care in the electronic medical record according to organizational standards and regulations
  • Coordinate care for abnormal results, triage, RN verification of vaccinations and injections, in-baskets, prior authorizations, and forms
  • Oversee clinical care supporting assessment, promotion, maintenance, and restoration of patient health and wellness
  • Follow nursing practices and protocols to minimize patient harm
  • Use information technology to communicate, manage knowledge, and mitigate errors
  • Monitor care outcomes and use improvement methods to improve patient-care quality and safety
  • Function within primary care inter-professional teams using open communication, mutual respect, and shared decision-making
  • Create an environment of belonging and inclusion and demonstrate problem-solving and conflict-resolution skills
  • Pursue continued professional growth and development
  • Participate in team huddles and assemblies
  • Give and receive feedback and integrate appropriate changes into practice

Requirements

What you’ll need
  • Graduate of an accredited school of nursing required
  • Current RN licensure in state of employment
  • AHA BLS certification
  • 1+ years’ experience as a Nurse in an ambulatory setting preferred
  • BSN or completion of BSN within 3 years of hire strongly preferred
  • Must maintain a current nursing license in the state of practice
  • Encouraged to obtain ambulatory care nursing certification
  • Expected to participate in or support quality, evidence-based practice and research of nursing governance initiatives
