About the role
Key responsibilities & impact- Use the nursing process to address patients’ physical, psychological, emotional, and social needs
- Integrate evidence-based practice, clinical expertise, patient and family preferences, and cultural beliefs into healthcare delivery
- Collaborate and communicate with clinic care providers regarding patient care and management
- Support timely communication between primary care providers and specialists
- Implement clinical care pathways and participate in quality improvement opportunities
- Provide evidence-based ambulatory nursing interventions during clinic visits, telephone encounters, and electronic communications
- Educate patients and designated care partners on preventive care, chronic disease management, medications, common illnesses, treatment, and recovery
- Document patient interactions and clinical care in the electronic medical record according to organizational standards and regulations
- Coordinate care, including abnormal results, triage, vaccination and injection verification, in-basket management, prior authorizations, and forms
- Oversee assessment, promotion, maintenance, and restoration of patients’ health and wellness
- Minimize patient risk by following nursing practices and protocols
- Use information technology to communicate, manage knowledge, and mitigate errors
- Monitor care outcomes and use improvement methods to enhance patient care quality and safety
- Function within ambulatory inter-professional teams and foster shared decision-making
- Provide safe, compassionate, and coordinated care based on patient preferences, priorities, needs, and cultural beliefs

Requirements
What you’ll need- Specialized Diploma from an accredited school of nursing required
- Current nursing license in the state of practice required
- Licensed Registered Nurse in Pennsylvania (RN - Licensed Registered Nurse_PA - State of Pennsylvania)
- Basic Life Support (BLS) certification from the American Heart Association (AHA)
- Knowledge of patient- and family-centered practice
- Cultural awareness and ability to connect effectively with people from diverse backgrounds
- Effective communication skills and ability to appropriately delegate to team members
- Professionalism, confidentiality, integrity, attention to detail, and time management
- Knowledge of HIPAA and ethical nursing conduct
- Knowledge of nursing and medical terminology
- Ability to assess ailments and injuries and triage appropriately
- Knowledge of child and adult education techniques and health literacy barriers
- Knowledge of pharmacology, medication administration, immunizations, and primary care medications
- Knowledge of patient assessment tools, nursing diagnosis, clinical judgment, and triage
- Knowledge of health information documentation methods
- Must attend and successfully complete EMR training and remain proficient in the EMR and its tasking function
- Ability to lift and carry 40–50 lbs.
- Frequent to continuous standing and walking; patient transport may involve walking 8–10 miles per shift
- Ability to kneel, bend, squat, and perform CPR as required
- Preferred: Bachelor’s Degree/BSN, completion of BSN, or appropriate advanced degree within 3 years of hire
- Preferred: 1 year of nursing experience in an ambulatory setting
- Encouraged to obtain ambulatory care nursing certification
- Expected to participate in or support quality, evidence-based practice, research, and nursing governance initiatives
