About the role

Key responsibilities & impact
  • Conduct comprehensive virtual patient evaluations, including detailed histories and telehealth-appropriate physical assessments
  • Order and interpret diagnostic studies when clinically appropriate
  • Manage acute urgent care complaints via telehealth
  • Assess risks associated with acute illness and determine need for in-person escalation
  • Diagnose acute and episodic conditions with the interdisciplinary team
  • Develop and prioritize differential diagnoses
  • Recognize complications and coordinate referrals to in-person care
  • Establish evidence-based treatment plans for virtual urgent care
  • Prescribe medications, diagnostics, and interventions according to telehealth clinical guidelines
  • Modify care plans based on follow-up, evolving symptoms, and patient needs
  • Document telehealth encounters in EPIC, including assessments, care plans, orders, and follow-up recommendations
  • Ensure documentation meets telehealth compliance, quality, and acuity-capture standards
  • Partner with clinical leadership, support staff, and healthcare-network colleagues to maintain continuity of care
  • Communicate with patients and families about virtual visits, treatment plans, and follow-up
  • Participate in telehealth-specific and organization training, education, and performance-improvement initiatives
  • Maintain CEUs, board certification, and professional memberships
  • Contribute to Jefferson’s virtual care model
  • Perform other duties as assigned

Requirements

What you’ll need
  • DNP, MSN/MSRN, or PA equivalent stated in the job-at-a-glance section
  • Master’s or Doctoral Degree for NP or PA
  • Family Medicine–boarded NP or PA
  • ER or Urgent Care clinical experience
  • Residence in Pennsylvania, New Jersey, or Delaware
  • Active licensure, or ability to obtain licensure, in Pennsylvania, New Jersey, and Delaware
  • Active Pennsylvania DEA required at the start of employment
  • No visa sponsorship
  • Prior telehealth experience preferred
  • Experience with high-acuity triage, virtual workflows, and remote diagnostic decision-making preferred
  • Maintain required CEUs, board certification, and professional memberships
