About the role

Key responsibilities & impact
  • Provide hands-on, nurse-led complex care to a young adult in their own home
  • Provide personal care, medication and clinical interventions
  • Support O2 monitoring, PEG, moving and handling, and specialist equipment
  • Manage tracheostomies, ventilation and specialist equipment as required
  • Monitor clinical observations and recognise deterioration
  • Follow detailed clinical care plans and protocols
  • Maintain accurate records and escalate concerns immediately
  • Support the individual to safely attend activities and enjoy meaningful experiences
  • Work as part of a dedicated nurse-led team under ongoing clinical oversight

Requirements

What you’ll need
  • A full UK driving licence
  • Experience in care or confidence undertaking clinical training
  • Excellent attention to detail and situational awareness
  • Ability to follow detailed clinical protocols consistently
  • Basic IT skills for communication and record-keeping
  • Genuine commitment to safe, person-centred care
  • Applicants must already have the right to work in the UK
  • Minimum availability of 10 hours and alternate weekends
