About the role

Key responsibilities & impact
  • Evaluate, plan, direct, and administer physical therapy programs to restore function, prevent disability, and maximize patient independence
  • Assess patient condition based on referring physician diagnosis
  • Develop and update plans of care in collaboration with patients and families
  • Provide prescribed exercises, hands-on care, and patient education
  • Implement individualized treatment plans using therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities
  • Reassess patients and modify therapy plans as needed
  • Communicate patient progress and physical limitations to supervisors, physicians, and team members
  • Provide age-specific and population-specific care using a patient/family-directed approach
  • Supervise in accordance with regulatory requirements when appropriate
  • Establish treatment plans with measurable goals and time frames
  • Provide cost-effective care according to billing guidelines
  • Complete documentation according to policies, standards, and regulatory requirements

Requirements

What you’ll need
  • Bachelor’s Degree in Physical Therapy
  • Sound clinical reasoning in synthesizing evaluation data and identifying treatment diagnoses
  • Knowledge of physical therapy techniques
  • Ability to provide safe, compassionate, and competent physical therapy treatments
  • Ability to communicate with patients, family, and co-workers in a friendly, supportive manner
  • Ability to travel to patient care assignments (Home Care only)
  • American Heart Association Basic Life Support certification in Pennsylvania upon hire
  • Physical Therapist license in Pennsylvania upon hire
  • Ability to lift and carry 40–50 lbs.
  • Frequent to continuous standing/walking
  • Ability to perform role-specific activities including kneeling, bending, squatting, and CPR
  • Preferred: Master’s Degree in Physical Therapy or Doctorate in Physical Therapy
  • Preferred: 1 year clinical experience for Home Care
