About the role
Key responsibilities & impact- Conduct comprehensive patient-entry assessments to determine anticipated length of stay, discharge needs, and resources
- Develop and implement individualized care and transition plans with the multidisciplinary team
- Coordinate safe and timely discharge planning, including post-acute authorizations, transportation, homecare, facility placements, and transfers
- Monitor patient progress through daily review and multidisciplinary rounds
- Identify and resolve barriers to discharge
- Advocate for patients and families and address clinical, educational, and psychosocial needs
- Collaborate with healthcare teams and coordinate patient or family conferences
- Refer patients and significant others to community-based services and resources
- Maintain knowledge of Medicare, Medicaid, payer benefits, reimbursement methodologies, and conditions of participation
- Educate patients and families through in-person, telephone, and electronic engagement to support discharge-plan adherence
- Communicate with physicians, patients, families, and team members regarding referrals, authorizations, medical necessity, and transition planning
- Initiate or follow up on medical assistance applications when necessary
- Deliver required regulatory notices within established timelines
- Manage resources and support performance-improvement initiatives to enhance patient care and system efficiency
Requirements
What you’ll need- Master’s Degree in Social Work
- Less than 1 year experience in an MSW internship program
- Ability to incorporate strategies for interacting with persons from diverse backgrounds
- Ability to set priorities to coordinate care plans efficiently
- Knowledge of computer applications and analytical tools
- Proven leadership skills
- Familiarity with EHRs: EPIC
- ACM (Accredited Case Manager) certification within 3 years or CCM (Certified Case Manager) certification within 3 years
- Colleagues working in the state of New Jersey only are required to hold an LSW or LCSW license upon hire
- Ability to lift and carry 25 lbs.; frequent sitting/standing and keyboard use; patient care providers may need to kneel, bend, squat, and perform CPR
