About the role

Key responsibilities & impact
  • Provide mental health assessment, diagnosis, treatment, and crisis intervention services for adult and/or child members
  • Coordinate patient care and assist patients in accessing community services and resources
  • Oversee utilization management, including service authorization and denial and appeal processes
  • Ensure discharge planning documentation is clear, concise, timely, and aligned with treatment team recommendations
  • Conduct clinical assessments and provide treatment interventions for patients and families according to treatment plans
  • Provide case management and care coordination services
  • Advocate for patients to help them achieve and maintain community stability
  • Liaise between patients, referral sources, and funders
  • Manage voluntary and involuntary mental health commitment processes under the Pennsylvania Mental Health Procedures Act
  • Participate in departmental and program quality improvement activities

Requirements

What you’ll need
  • Master’s Degree in Social Work, Counseling, or related field
  • Ability to remain patient and calm in stressful situations
  • Knowledge of community resources
  • Understanding of how to make appropriate referrals to community and governmental agencies
  • Understanding of clients’ psychosocial and medical needs to formulate an integrated, comprehensive treatment plan
  • One of the following Pennsylvania licenses: LCSW (Licensed Clinical Social Worker), LPC (Licensed Professional Counselor), or LSW (Licensed Social Worker)
  • Ability to lift and carry 40–50 lbs.
  • Frequent to continuous standing/walking
  • Ability to perform role-specific activities including kneeling, bending, squatting, and CPR
  • Availability for night shifts, weekend and holiday rotation
